SpaceX, did Musk get it wrong?

SpaceX Starship suffered a Rapid Unscheduled Disassembly (RUD) 4:04 after launch.
Even though the SpaceX Starship exploded at four minutes into its flight, SpaceX pronounced it a unmitigated success as they were able to get loads of telemetry to make the next flight better. However, was the problem a bit more down to earth than that, perhaps SpaceX should look at more concrete examples of what went wrong?
